New Student Registration Information
 
We are so pleased that you are interested in enrolling your child or children at Sibley East.  We look forward to meeting you.  We are very proud of the various services we extend to our students and families.  
 
If you have decided to join us, you will need to complete a number of forms, which are required for the enrollment process.  The forms are either required by the State of Minnesota or the Sibley East Schools. 

Required Forms

Click the "Required Forms" link to your left to begin the process.  You will need to download and print the forms. Online submission is not available at this time.  You may, of course, also pick up the required forms at either school office.

NOTE: If at all possible, please present your forms in person at either the elementary or secondary office.  You can present the forms in your home community even if your child attends school in the other.  In person is preferred because office staff often has to assist the user in various areas of the forms.  

If you are unable to present the forms in person, you may, of course, mail the forms to either school office.  If errors are present, office staff will contact you to resolve any questions.

Immunization Requirements

The State of Minnesota requires certain immunizations at various times in your child's development. At the time of immunization, ask your physician for a printout of your child's immunization record, which you must then present to the office staff at either campus. Be sure to get this printout prior to leaving your clinic.

  • It is very important that you read the backside of the informational form. It will provide the steps you must take if your child is unable to be immunized or if you choose not to immunize your child.
